Differences: trans*, inter*, and non-binary*

People who identify as non-binary do not see themselves as exclusively male or exclusively female. Non-binary identities are diverse. They can lie between, beside, beyond, or independent from the categories “male” and “female.” Non-binary is also an umbrella term that can encompass various identities (e.g., genderfluid, agender, bigender, demigender, genderqueer, etc.).

Many non-binary people see themselves as part of the trans* community because their gender identity does not match the sex they were assigned at birth. Others reject using the term “trans” for themselves.

The correct form of address

It is important to address non-binary people using their preferred pronouns Find out more about this topic under “Pronouns”. Preferred pronouns can vary from person to person.

Non-binary people use a wide variety of pronouns:

  • they/them
  • xier/xiem/xien
  • dey/deren/deyn (German), xe/xem (English)

Some people use different pronouns interchangeably, or do not use pronouns at all, or only want to be addressed by their first name.

The most important thing is that there is no “right” or “wrong,” only what is right for the person themselves.

If you are unsure, it is best to just ask which pronouns they prefer, or start the conversation by stating your own pronouns.

Using incorrect pronouns, even if unintentional, can be hurtful because it puts the person’s gender into question or makes it invisible.

In everyday life, it is also helpful to try to use gender-neutral language, such as “chairperson” or “representative" (in German, for example, " „die Person“, „Kolleg*in“ or „jemensch“).

Contraception & STI prevention for non-binary people

Some non-binary people want medical transitions (e.g., hormone therapy, surgery), others do not.

  • People with a penis can produce sperm despite estrogen therapy.

  • People with a uterus can continue to ovulate while on testosterone therapy..

 

This means: non-hormonal methods such as condoms, IUDs, or diaphragms work as usual. Hormonal methods can also be used—which ones are suitable depends on the organs, not on gender identity. For non-binary people with a uterus, hormonal contraception can also be helpful in stopping periods or making them more regular.

Important: Hormonal contraception is not a substitute for hormone therapy. The estrogen used differs in form and dosage.

STI prevention

Trans* and non-binary people worldwide are at increased risk for STIs    Robert Koch Institut & Deutsche Aidshilfe. (2023). How they protect themselves from these infections may depend on certain factors, such as gender-affirming surgery. Depending on their genital anatomy, they may use condoms or femidoms (aka female condoms).

Legal status and health of non-binary persons

In Germany, since 2019, it has been possible to specify not only “male” or “female” but also ‘diverse’ or “none” as the gender entry in civil status (vital) records.

Many medical systems are organized in a binary way. There is a great need for doctors and health professionals who are aware of and sensitive to the needs of non-binary people. This unmet demand can have serious consequences: from a lack of preventive care to unnecessary psychological stress and increased suicidal tendencies due to minority stress.

Non-binary people also have a higher risk of mental illness.
